The Sequoia Healthcare District in honor of our 70th year of service to our residents will offer a new innovative and exciting new project, called 70 Strong, which will connect older adults to health and community services.
Targeted at those aged 70 and older, the program will connect residents through 4 informational vehicles: a new robust website, a phone contact system, community kiosks and through in-home visits with a social worker.
All residents of the District are eligible to use the service and will be offered free with all costs covered by SHCD. 70 Strong will be administered through a partnership with Peninsula Family Services, a leading social service organization serving older adults in San Mateo County.
The goal is to have the program operational by January 1, 2017 and to serve 4,000-5,000 residents.
