(May 24, 2016) The Association of California Healthcare Districts (ACHD) is pleased to announce that Camarillo Health Care District received the Healthcare District of the Year award. The award was presented at ACHD’s Annual Meeting on May 5th, 2016, in Monterey, CA.
To be recognized as Healthcare District of the Year, a District must have implemented programs and/or services that have yielded direct and measurable benefit on the health and well-being of its residents. Camarillo Health Care District is dedicated to providing a wide range of community health, wellness, and safety services, and is recognized statewide as an innovative leader in person-centered community health care.

Camarillo Health Care District’s innovative “Changing Aging” campaign began when the District resolved to change key paradigms about aging. The initiative focused on collecting data that would assist the District in creating targeted programs to change the philosophy and practice of healthcare’s relationship to aging. The District also implemented the “Partners at Home” program, which prototypes innovative models of partnerships between physician groups, hospitals, accountable care organizations and health plans seeking community-based care “extenders” who work with patients that are fragile, socially complex, and costly. The Camarillo Health Care District remains focused on implementing services and programs that improve community health and well-being, while offering added value to the community.
California Healthcare Districts respond to the specialized health needs of California communities. Voters created 78 Healthcare Districts to fulfill health local care needs. Of these, 54 serve the state’s rural areas. Healthcare Districts provide access to essential health services and are directly accountable at the community level. As a result, tens of millions of Californians have been able to access care that would otherwise be out of reach.
